At Horizon View Medical Clinic, we are dedicated to assisting our patients with their prescription needs. To ensure efficient service, we provide a dedicated prescription line managed daily by our staff.
Patients and pharmacists alike have access to our dedicated prescription line, and pharmacy requests can also be faxed or emailed to our team. Our team diligently handles requests throughout the day, obtaining physician approval and processing orders promptly.
For prescription requests, please include the patient's name, date of birth, required medication, ordering physician, and pharmacy contact information. All prescriptions will be handled with utmost care and either called in or written on a physician order form for your convenience.

Patient Financial Responsibility:
At Horizon View Medical Clinic, we strive to provide exceptional care to our patients while ensuring transparency in our financial policies. Please note the following regarding payment responsibility:
The patient (or the patient’s guardian, if a minor) holds ultimate responsibility for payment for their treatment and care.
We are delighted to assist by billing contracted insurers. However, patients are required to furnish accurate and up-to-date insurance information. Any charges incurred due to incorrect or outdated information are the responsibility of the patient.
Patients are accountable for copays, coinsurance, deductibles, and any procedures or treatments not covered by their insurance plan. Payment is expected at the time of service, and we offer the convenience of accepting cash, check, and most major credit cards at our office.
Patients may be subject to additional charges at the discretion of Horizon View Medical Clinic. These charges may encompass, but are not limited to:
Fees for returned checks
Charges for missed appointments without 24 hours' notice
Fees for extensive form completion
Any expenses related to the collection of patient balances

At Horizon View Medical Clinic, we recognize that unforeseen emergencies or obligations may occasionally lead to missed appointments. However, failing to notify us of a cancellation deprives other patients of needed care and appointments. It's a situation where the person who misses out could easily be you!
If an appointment is not canceled at least 24 hours in advance, the following no-show fees will apply:
$50 for office visits
We understand that certain unavoidable circumstances may necessitate a cancellation within 24 hours. In such cases, fees may be waived, but only with approval from management/physicians.

I acknowledge that Horizon View Medical Clinic is not obligated to agree to requested restrictions. I understand that I can revoke this consent in writing, except where the clinic has already taken action based on it. I also acknowledge that by refusing to sign or revoking this consent, the clinic may refuse treatment as permitted by applicable regulations.
Furthermore, I understand that Horizon View Medical Clinic reserves the right to change its notice and practices, provided it adheres to regulatory requirements. If such changes occur, the clinic will send a revised notice to the address I've provided, whether by U.S. mail or email, if agreed upon. I am aware that I have the option to restrict the use or disclosure of my health information.
